In the event of a disaster, please type:

* your name

* your location and new location if you are relocating

* telephone number where to reach you now, and in the future if you are relocating

* status of all family members

* full names & status of others you may have with you

* all supplies that you need

* etc.

Check back daily until ALL basic family members are accounted for.
